Helps adults struggling with mental health and/or substance abuse challenges. CORE provides clients with access to mental health and supportive services. Staff use a strengths-based approach to help clients regain control of their lives.

Services offered by CORE are voluntary and may include, but are not limited to:

  • Medication Support​
  • Entitlement Support through SMART Program (GA, SSI, SSDI, etc.)
  • Linkage to Primary Health Care
  • Individual Therapy
  • Support Groups
  • Case Management
  • Outreach and Advocacy
  • Community Wellness Centers
  • Referral services to programs to assist with interim and long-term housing

Program Delivery
Eligibility: Individuals 18 and older who receive Medi-Cal are eligible for Specialty Mental Health Services in Sacramento County.
Languages: Translation services are available for multiple languages upon request. American Sign Language Interpreter available on request, English
Application Process: Individuals can self-refer to a CORE program by reaching out to the Sacramento Access Team for a screening or by walking into a CORE provider site during the designated walk-in hours for an assessment if capacity allows. Call to confirm walk-in availability.
