Eligibility:

Low-income residents of Sacramento with:
- Medi-Cal Fee-for-Service
- Medi-Cal Managed Care
- Healthy Partners enrollment
- Medicare
- Medi-Medi (eligible for both Medicare and Medi-Cal)
Uninsured low-income individuals with a medical need may go in for services and enrollment assistance.
Must not be receiving care from another medical provider.
