Description

A Medi-Cal based program that pays for in-home care services to allow eligible aged, blind, and people with disabilities to live safely at home. In-home services are an alternative to out-of-home care, such as nursing homes or board and care facilities.

Services may include:

  • Housecleaning
  • Meal preparation
  • Laundry
  • Grocery shopping
  • Personal care services (bowel and bladder care, bathing, grooming, etc.)
  • Accompaniment to a medical appointment
  • Protective supervision

Program Delivery
Eligibility: Must meet the following requirements:
- Must live in Sacramento County
- Must be a US citizen or a legal permanent resident of California
- Must be 65 years of age or older, or blind or disabled of any age
- Must have Medi-Cal
- Must live at home or an abode of one's choosing
- Must be unable to live at home safely without assistance
- Must submit a completed Health Care Certification Form (SOC 873)
Languages: Services provided in multiple languages, American Sign Language Interpreter available on request, Armenian, Hmong, Laotian, Mien, Punjabi, Russian, Spanish, Vietnamese, English, Chinese
Application Process: Call or submit an application via mail or fax. Once the requirements and Medi-Cal eligibility are met, a social worker will be assigned to the case to conduct a needs assessment.
