 Description

Provides intensive in-home-based services to youth up to age 21 who are experiencing behavior difficulties that place the youth at risk for psychiatric hospitalization or loss of housing placement. Aims to improve behaviors in the home to help youth maintain housing placements and prevent hospitalization or residential stays.

Services include:

Program Hours:
Sacramento County Access Team: Seven days a week, 24-hours;
Office: Monday through Friday, 8 am - 6 pm

Program Delivery
Eligibility: Children and youth up to age 21 with full-scope Medi-Cal and their families.

Must also be:
- Receiving Medi-Cal outpatient (mental health) services.
- Living in/being considered for/or stepping down from a Level 12-14 group home/Short-Term Residential Therapeutic Program OR have been assessed in a hospital for psychiatric needs in the past 6 months OR have been in a psychiatric hospital in the last 24 months.
Languages: Most languages by arrangement, American Sign Language Interpreter available on request, Spanish, English
Application Process: Referral required from the Sacramento County Access Team.

Program Fees: Accepts full-scope Medi-Cal.
