Description
Case management services are provided to individuals who are 18 years or older, and who have substance use or co-occurring psychiatric and substance use disorders. Case managers provide routine or intermittent low-intensity support needs, including:  linkage and access to treatment services, assistance with problem solving and attainment of  community resources, advocacy for entitlements and housing, and other supports designed to improve quality of life in order to reduce incidences of relapse and improve daily level of function.

Program Delivery
Eligibility: Ages 18+; Must meet the DSM 5V criteria for a substance disorder and/or co-occurring disorder.; At least 3 of the following: 1) History of needing continuous supportive treatment; 2) Current housing or homelessness increases risk of relapse; 3) Lack of financial resources; 4) Other condition(s) increases risk of relapse; 5) Involved in or appropriate for specialized services, i.e. DCF, AIC, probation
