Outreach program seeks to identify, engage, refer and link older adults, ages 55+ to community treatment options and resources to help seniors remain in their homes and communities. Program works to keep seniors independent by identifying needed medical, substance use, or mental health treatment and counseling services. Program staff meet with clients, providers and caregivers to coordinate referrals to essential benefits and resources in the community.
