Program Delivery
Eligibility: Stanislaus County residents with proof of residency such as a current and valid motor vehicle registration, or a current rent or mortgage receipt or utility bill in patient's name bearing current address within the county.
Languages: English and Spanish.
Application Process: All applicants MUST schedule an appointment by calling 209-558-7191. Walk-in clients CANNOT be accommodated.
Payment methods:
Program Fees: A non-refundable processing fee. For Medi-Cal beneficiaries and their caregivers, fees will be half-price. Please call for current prices.
Documents Required: * Written documentation by their licensed physician warranting the use of medical marijuana and resides in the county where the application is submitted;
* Proof of residency such as a current and valid California motor vehicle registration, current rent or mortgage receipt or utility bill in patient's name bearing current address in the county;
* Government issued photo identification such as a current and valid driver's license or identification card issued bye the DMV bearing patient's name and current address; and
* Medi-Cal eligibility (if applicable) must be submitted in person at time of appointment.
