Description

Provides intensive outpatient mental health services to youth and young adults. Helps individuals 16-26 years of age with Serious Emotional Disorders (SED) requiring a high level of support. Individuals often have multiple hospitalizations or encounters with crisis services. Sometimes they are unable to leave their home due to mental health issues. Offers the following services:

  • Individual and family counseling
  • Case management
  • Psychiatric care and medication support
  • 24-hour crisis support
  • Referral & linkage to additional care

Eligibility: This program is for youth and young adults ages 16-26 who are:
-Coming out of multiple hospitalizations or CSU stays
-Justice involved (Juvenile Hall/Ranch/Jail) and/or on parole/probation due to behaviors connected to their mental health
-At risk for losing placement due to their behaviors
-Experiencing high suicide ideation, attempts, risk factors and/or escalating self-harm
-Involved in the foster care/DFCS
-Experiencing high acuity diagnoses and co-occurring diagnoses needing additional support
-Served by multiple systems of care
Languages: Spanish, English
