Program Delivery
Eligibility: Person HIV positive; Net countable income must be at or below 500% Federal Poverty Level; Must reside in Connecticut
Application Process: Individual who wants to apply for CADAP must complete and submit a CADAP application; Applications may be completed by the applicant or the applicant’s HIV case manager, social worker, clinician, or anyone else working in an official capacity on the applicant’s behalf or enroll online at: https://ct.enroll.lh.primetherapeutics.com
