Eligibility: Must be homeless or low-income. Must be able to provide proof of income and expenses for the month.
Languages: Spanish, English
Application Process: Call or walk-in to schedule a required appointment.
Payment methods:
Program Fees: $10 fee, must be debit card payment.
Documents Required: A prescription from the last 24 months that contains the Pupillary Distance (PD) measurement. Proof of income and expenses for the month. Proof of mailing address where the glasses can be sent.
Normal Wait Time: 10 minutes for appointment.
