Description

IDENTIFICATION DOCUMENTS: Partners with Identity Access Project to assist with obtaining out of state birth certificates or MD birth certificates and IDs only. Assistance is available once in a lifetime. Individuals must schedule an appointment with MVA, and a voucher will be given near the time of MVA appointment. Birth certificates are available for those born in Maryland, Washington D.C., Virginia & Pennsylvania.

Eligibility: City residents; low-income persons in need.
Application Process: Individuals can walk-in for assistance.

Program Fees: None.
Documents Required: For birth certificate, individuals must have any of the following acceptable forms of ID MTA ID, prison badge or ID, BGE, phone bill, medical card or record, lease, pay stub or social security card etc. For MD ID, individuals must have original birth certificate, original social security card, and two pieces of mail or one piece of mail if in a state or federal government operated program such as a shelter, substance use treatment program, etc.
Service Area: All Baltimore City zip codes; NO county residents. Birth certificates are available for guests born in Maryland, Washington D.C., and Virginia.
