Offers an alternative to a nursing home or institutional living. Helps put supportive services in place to allow medically fragile individuals to stay at home. Works together with a social worker and a nurse to determine the services the client needs. Offers regular visits to clients to provide ongoing support, information and referrals.

Eligibility: Residents of all ages with major functional difficulties who need extensive personal care or nursing services and are eligible for full-scope Medi-Cal.
