Description

Intensive in-home therapeutic intervention only for New Britain, Bristol, Cheshire, Meriden, Simsbury, Newington, Wallingford, or Wethersfield families with high risk children through age 5 who have serious emotional disturbance, developmental and learning and abuse/neglect issues in the home. Provides hands on care coordination and case management to connect with community based services and address barriers to service access. Coordinator develops a family-driven treatment plan of supports and services, reflecting family culture, goals, priorities, strengths and needs. Also provides community consultation and training for early childhood and adult providers working with a family with significant environmental or psychosocial risk, child emotional/behavioral problems or developmental or learning questions.

Program Delivery
Eligibility: Resident of New Britain, Bristol, Cheshire, Meriden, Simsbury, Newington, Wallingford, or Wethersfield; Children through age 5 exhibiting emotional and behavioral disturbances or who have significant developmental delays or who have parents with multiple challenges that impeded their ability to support and nurture children; Child and all members of the family are eligible for services; No exclusion criteria (i.e. parental substance abuse, severe parental mental illness, foster care, homelessness, undocumented status)
