Description
Connects residents of Elk Grove who are currently homeless (living outdoors or in a vehicle) to services and resources. Homeless Navigators work to identify and build relationships with people experiencing homelessness. They assess each case in order to meet the person's needs, address barriers they may face, and identify possible housing options.
Services include:
- Assistance finding housing
- Assessments for shelter/housing
- Connection to mental health resources and other social services
- Homeless verification letters
- Assistance with obtaining IDs, financial benefits, and mobile phones
- Transportation to important health appointments, housing resources, mental health support, etc.
The program does NOT serve individuals who are at risk of homelessness. Individuals must be living outdoors or in a vehicle to be eligible.
