Provides primary and preventative health care services to low-income, undocumented adults residing in Sacramento County. Services are delivered at the Sacramento County Health Center and include:
  • Primary Health Care (including health and behavioral health services)
  • Women’s Health
  • Preventative Care (including immunizations and flu shots)
  • Treatment of chronic conditions
  • Lab and radiology
  • Prescription medications (low cost)

Program Delivery
Eligibility: To be eligible for services, individuals must:
- Be19 years of age or older
- Be Sacramento County residents
- Meet income requirements (income at or below 138% of the Federal Poverty Level)
- Have certain restricted scope Medi-Cal coverage (with applicable Aid Code)
- Do not have health insurance and do not qualify for other healthcare programs
Languages: Translation services may available for multiple languages upon request, Spanish, English
Application Process: If eligible, call Member Services. Healthy Partners staff will enroll an individual if they meet requirements. Staff will also provide information and schedule a medical appointment to establish care.
Payment methods:
Program Fees: Primary care: No fee.
Medications: Fees vary by medication, but are considered low cost.
Lab/Diagnostic Services: No fee.
Documents Required: Photo ID, proof of Sacramento County residence, and proof of income.
