Provides comprehensive reentry care coordination services for adults transitioning out of incarceration. Peer Support Case Manager provides Enhanced Care Management and Care Coordination. Services include:

  • Care plan development
  • Coordination of services
  • Referrals to medical, dental, mental health and substance use treatment services
  • Assistance with accessing other services depending on one's needs, such as job searches.
