Program Delivery
Eligibility: Adult residents of Santa Clara County ages 18-59 who meet one of the following criteria:
-Involved in the criminal justice system
-An older adult age 60 and over with a history of severe mental illness (SMI) AND are high service needs with no access to other community-based services
Languages: Other languages available through interpreter, Spanish, English
Application Process: Referrals are accepted through Santa Clara County Courts and other Forensic Service Providers within Santa Clara County.
Program Fees: No fee. Accepts MediCal.
Documents Required: Proof of insurance.
