ASSESMENT EVALUATION & REVIEW SERVICES (AERS): Provides assistance to functionally disabled individuals who are at risk of being placed in an institution. Nursing staff conducts a comprehensive evaluation to identify services that may be able to support the individual at home and allow them to remain in the community, or in the least restrictive environment, while functioning at the highest possible level of independence and personal well-being. Team consists of Community Health nurses; a physician and a psychologist are consulted for PASRR assessments only (Preadmission Screening and Resident Reviews).

Program Delivery
Eligibility: Must meet both medical and financial eligibility requirements. Medical: at risk of nursing home placement or institutional care. Financial: Medicaid recipients or be eligible for Medicaid within 6 months of placement in a nursing facility, or a Priority #1 on the Community Options Waiver registry. Limited programs available through MAC and DSS for individuals without Medicaid eligibility. Adults Teens Children

Application Process: Referrals are accepted from any concerned individual by phone, fax, mail, or in-person through the MAC center. MAC does a screening and forwards eligible candidates to AERS. Referrals for Pre-Admission Screening and Resident Reviews generally come from hospitals, nursing facilities, private physicians, or other healthcare providers.
