Digital Vaccine Record (DVR) can include:

  • California Immunization Record: includes all vaccinations reported by one's healthcare provider or pharmacy to CAIR.
  • COVID-19 Vaccine Record: includes all COVID-19 vaccine doses and dates.

Application Process: Visit the website to fill out the Digital Vaccine Record Request Form. A link to the vaccine record and a QR code will be sent to the email or phone number associated with the vaccine record.

To update incorrect personal or contact information, submit a request on the website to the virtual DVR Assistant. If doses on a record are missing or incorrect, contact the healthcare provider to have them update the records. For additional assistance, send an email.
