Program Delivery
Eligibility: Ages 0-18 years. Referral agent must fill out a 2 page intake form.
Application Process: Does NOT accept direct referrals from parents. Only accepts referrals from agencies and other providers.
Program Fees: Third party insurance. Medicaid. New York State Crime Victims' Assistance may be available.
Documents Required: Intake form must be completed.
