Description

CalAIM, also known as the Medi-Cal Transformation Initiatives, works with partner agencies to provide community-based services and support that address health-related social needs.

Community Support offerings vary for each managed care plan within each county. Services are also dependent on partnerships between each managed care plan and its service partners.

Medi-Cal managed care health plans may offer one or more of the following services to qualifying members:

  • Housing transition navigation services
  • Housing deposits
  • Housing tenancy and sustaining
  • Short-term post-hospitalization housing
  • Recuperative care (medical respite)
  • Respite services
  • Day habilitation programs
  • Nursing facility transition/diversion to assisted living facilities
  • Community transition services/NF transition to a home
  • Personal care and homemaker services
  • Environmental accessibility adaptations (home modifications)
  • Meals/medically tailored meals
  • Sobering centers
  • Asthma remediation
  • Dental Care

Program Delivery
Eligibility: Members in Medi-Cal Managed Care with complex physical behavioral, developmental, and social needs. Members do not have to be eligible for Enhanced Care Management (ECM) to be eligible for one or more Community Support.
Community Supports are optional for Medi-Cal members and Medi-Cal managed care health plans. All Medi-Cal managed care plans (MCPs) are encouraged to offer as many of the 14 pre-approved Community Supports as possible.

Application Process: Members can contact their health plan and ask to see if they qualify by calling the number on the back of their insurance card or by having their health and social services provider submit a referral to their health plan.
Medi-Cal health plans will determine whether a member is qualified to receive Community Supports (CS). If qualified, the health plan will either advise the member to contact a specific partner agency, or a Community Supports provider will reach out to the member to see if they would like to receive the benefit.
