Confidential health survey administered to help CDPH improve health services in the community. Survey may include questions about an individual's medical history, use of medical and social services, etc. Survey runs from June 1st to April 15th every year. Participants who complete the survey receive a $50 - $75 gift card as a token of appreciation.

Only individuals contacted by CDPH can take the survey. Individuals cannot volunteer to take the survey.

Program does not serve San Francisco and Los Angeles Counties.
