Please note: HEART is a behavioral health program, not a housing program.

The Homeless Engagement and Response Team (HEART) conducts outreach in local shelters and encampments to meet unhoused individuals where they are. Staff work to establish rapport, determine readiness for help, screen for mental health and substance use disorders, and provide warm handoffs to appropriate behavioral health providers for ongoing treatment and support.

HEART coordinates with Sacramento County's Department of Homeless Services and Housing, Department of Human Assistance, and Sacramento City's Department of Community Response to provide services.

Eligibility: Unhoused individuals needing mental health or substance use treatment who are not already connected to Sacramento County Behavioral Health Services.
Languages: English
Application Process: Call, email, or submit a HEART Referral Form through the website. Individuals can self-refer or be referred by someone else.

For phone and email referrals, leave one's name, phone number, and a brief message describing what services are being sought.

Once submitted, HEART staff will review the referral. Field staff will then attempt to locate the individual and begin the outreach and engagement process.
