Program Delivery
Eligibility: 1. Be enrolled in a Medi-Cal managed care plan that Pair Team contracts with in this region — e.g. Inland Empire Health Plan (IEHP) and

2. Meet criteria for at least one ECM Population of Focus, including:
- Adults and children/youth experiencing homelessness
- Adults and children/youth at risk for avoidable hospital or emergency department use
- Adults and children/youth with serious mental health and/or substance use disorder needs
- Adults at risk for long-term care institutionalization
- Adult nursing facility residents transitioning to the community
- Adults and youth transitioning from incarceration, or released within the past 12 months
- Children and youth involved in child welfare, or with complex physical, behavioral, or developmental needs
- Pregnant and postpartum individuals at high risk of adverse outcomes
