Offers Medi-Cal members experiencing homelessness or at risk of experiencing homelessness assistance in finding, applying for, and securing housing. Assistance includes assistance in searching and applying for housing, housing tenancy support, housing sustaining services, and connection to resources. Assistance does NOT include eviction avoidance.

Exact services received vary by individual client need, and Managed Care Plan approval, but may include:

  • Housing Transition and Navigation Services: Assistance with locating and obtaining housing, resources, referrals, and landlord mediation services
  • Housing Deposit Assistance: Utility/security deposits, and/or other eligible costs related to securing permanent housing

Helps individuals and families follow a path to attainable and sustainable housing. Individuals must participate in navigation services that come with any funds awarded. UWCCR services include working with a Housing Navigator who provides housing search assistance, identifies housing options, and assists in the application process. During this time, the Housing Navigator will identify barriers and housing needs for the individual or family and secure the funding for security deposit costs. Housing Sustainability Providers will also be able to assist participants in maintaining stable housing.

Program Delivery
Eligibility: To be eligible, an individual/family must meet the following eligibility requirements:
- At least one member of the household must be a Medi-Cal member with either Health Net, Molina Healthcare, Kaiser, or Anthem Blue Cross.
- Live in Sacramento County.
- Experience or be at risk of homelessness.
- Have a chronic medical condition (e.g., rheumatoid arthritis, COPD, mental health diagnosis, substance use disorder, etc.)
- Have a stream of income that allows them to financially cover the cost of rent (consider all sources that can be put towards the cost of rent, including vouchers and other supports).
Languages: Translation services are available for multiple languages upon request, Japanese, Spanish, English, Chinese
Application Process: Call or email to learn more. Individuals can also submit a request for services online, along with submitting all relevant documentation needed to determine eligibility.

Once an application is approved, clients must participate in ongoing services, including at least one in-person visit with their Housing Navigator per month.
Payment methods: Accepts Medi-Cal
Program Fees: No fee for service. Clients must have a stream of income in order to maintain housing payments.
Documents Required: - Documentation showing that housing is in jeopardy or has been lost (e.g. verification letter from a 3rd party housing non-profit, eviction notice, 30 or 60-day notice, Homeless Certification, Section 8 Voucher, HMIS listing) - Proof of Income (e.g. CalWorks, SSI, Social Security or Pension, W-2, 1099 Form, Payroll Stub) - Medi-Cal ID and Health Plan Member information.
Normal Wait Time: 2-3 business days for initial call back. Referral process once all documentation has been received takes 3-4 weeks for approval.
