Helps with education aimed at building skills to live independent and satisfying lives and maintain stable housing. Aims to keep the members' current home environment and works to remove any barriers that might jeopardize their staying within their homes.

Designed to guide members in addressing the unique needs of Medi-Cal Managed Care Plan members and the factors that strongly influence people’s health and overall wellness, including working on needs such as housing, food, and financial stability.

Program Delivery
Eligibility: To be eligible, an individual/family must meet the following eligibility requirements:
- At least one member of the household must be a Medi-Cal member with either Health Net, Molina Healthcare, Kaiser or Anthem Blue Cross.
- Live in Sacramento County.
- Must have current secured housing.
- Have a chronic medical condition (e.g., rheumatoid arthritis, COPD, mental health diagnosis, substance use disorder, etc.).
- Have a stream of income that allows them to financially cover the cost of rent (consider all sources, including vouchers and other supports).
Languages: Translation services are available for multiple languages, Japanese, Spanish, English, Chinese
Application Process: Call or email in order to learn more. Individuals can also submit a request for services online, along with submitting all relevant documentation needed in order to determine eligibility.

Once an application is approved, clients must participate in ongoing services, including at least one in-person visit with their Housing Sustainability Provider per month.
Payment methods: Accepts Medi-Cal
Program Fees: No fee for service. Clients must have income in order to maintain housing payments.
Documents Required: - Proof of Housing - Proof of Income (e.g. CalFresh, CalWorks, SSI, Social Security or Pension, W-2, 1099 Form, Payroll Stub) - Medi-Cal ID and Health Plan Member information.
Normal Wait Time: 2-3 business days for initial call back. Referral process once all documentation has been received takes 3-4 weeks for approval.
