Description

Offers Medi-Cal members experiencing homelessness or at risk of experiencing homelessness assistance in finding, applying for, and securing housing. Assistance includes assistance in searching and applying for housing, housing tenancy support, housing sustaining services, and connection to resources. Assistance does NOT include first or last month's rent.

Helps individuals and families follow a path to attainable and sustainable housing. Individuals must participate in services that come with any funds awarded.

Program Delivery
Eligibility: To be eligible, an individual/family must meet the following eligibility requirements:
- Be a Medi-Cal recipient with Anthem, Molina Healthcare, Partnership Healthplan of California, Kaiser Permanente, or Health Net.
- Live in a county within the service area.
- Have at least one chronic physical, behavioral, or developmental health need with the inability to self-manage.
- Experience or be at risk of homelessness. At risk of homelessness is generally defined as those who will lose primary residence within 14 days, have nowhere else to go, and have no resources or support networks to obtain permanent housing. Individuals are encouraged to share their FULL Housing History and Current Housing Status in the application to ensure the reviewer can accurately assess eligibility.

Application Process: Interested individuals may complete the LSS Medi-Cal Transformation Referral Form on the LSS website. A person may also refer another individual but will need the name, medical record number, and birthdate of that individual.
Payment methods: Accepts Medi-Cal
Program Fees: No fee for services. Clients must have a stream of income in order to maintain housing payments.
Documents Required: Medi-Cal ID and Health Plan Member information.
Normal Wait Time: Approximately 2 weeks for health insurance authorization.
