Description

New parents program is a service for families in the Greater Bridgeport and Norwich area with new babies to help ease their transition from hospital to home. Family Bridge offers 3 at-home visits from Registered Nurses and certified Community Health Workers, to help new moms with their infant care, provide moms a wellness check and provide education about bathing, feeding, sleeping, safety, and assistance with more resources.

Program Delivery
Eligibility: Families who gave birth at Bridgeport Hospital or St. Vincent’s Hospital and reside in Bridgeport, Easton, Fairfield, Milford, Monroe, Shelton, Stratford, Trumbull and Norwich, will be referred to this program or may enroll
Application Process: Contact Family Bridge Program Support Specialist, Pediatrician or nurse at Bridgeport or Saint Vincent's Hospital where birth took place
Program Fees: No fee
Documents Required: Documents provided by clinician
