Eligibility: Adults with severe mental illness and/or substance use disorders who are experiencing chronic homelessness.

Prioritization for special populations of chronic homelessness:
- Justice Involved
- Elderly 55+ ​
- Former Foster Youth
- Fleeing Domestic Violence​,
- Community Assistance, Recovery, and Empowerment (CARE) Act participants
