Eligibility:

Adults with severe mental illness and/or substance use disorders who are experiencing chronic homelessness.
Additionally, families qualify if at least 1 caregiver meets eligibility. Must meet all 4 requirements: 1. Experiencing Chronic Unsheltered homelessness: living on the street, in a park, car, tent, etc. – Couch surfing is not unsheltered homelessness; 2. Client does NOT require nursing-level medical needs; 3. Client qualifies and agrees to participate in behavioral health treatment services; and 4. Client can perform Activities of Daily Living (ADLs). ADLs include being independent with mobility, hygiene, housekeeping, laundry, medications, and basic meals.
Prioritization for special populations of chronic homelessness: - Justice Involved - Elderly 55+ - Former Foster Youth - Fleeing Domestic Violence, - Community Assistance, Recovery, and Empowerment (CARE) Act participants
