Description

The Housing and Disability Advocacy Program (HDAP) is designed to assist individuals who are experiencing literal homelessness and who are also unemployable due to a disability. HDAP provides case management services and benefits advocacy to assist with the application or appeals process for disability benefits.

Housing-related financial assistance and wraparound supportive services provided by HDAP include, but are not limited to:

  • Interim Shelter Assistance
  • Rental Assistance
  • Housing Navigation
  • Case Management
  • Security Deposits
  • Utility Payments
  • Moving Costs
  • Legal Services
  • Referrals to Credit Repair Services

Program Delivery
Eligibility: To qualify as participants, homeless individuals in Sacramento County must:
- Be identified as highly vunerable by a CAS assessor.
- Be at least 18 years of age or older.
- Be experiencing chronic homelessness.
- Have a mental health or physical disability that is expected to last over 12 months, or be aged 65+.
- Not have an approved or pending SSI/SSDI application.
- Not have someone already representing them with the SSI/SSDI application process.


Note: A Homeless Certification is not required to verify homelessness for HDAP. A Social Worker can verify homelessness by site verification.

Languages: Telephone interpreting services, Laotian, Mien, Spanish, Thai, English
Application Process: Complete a Homeless Conversation Tool (HCT) assessment with a CAS assessor. Once HCT is on file, Sacramento Steps Forward (SSF) referral team will monitor waitlist, assess eligibility, and make referrals. If HCT assessment is not on file, dial 2-1-1 to complete HCT. 


Please note, 211 does not have access to verifying eligibility or checking the waitlist status.
