Ensures all primary care providers (PCPs) have access to real-time psychiatric consultation and resource support for their patients under the age of 22 years presenting with mental health and/or substance use concerns, regardless of insurance. Telephonic care coordination is also available to youth and families after the provider contacts the program. Regional partners: Hartford Hospital, Wheeler Clinic, and Yale Child Study Center.

Eligibility: Youth up to age 22; Primary care providers and pediatricians must call their regional Hub Team for information and referral; Website lists coverage areas for each Hub Team
Application Process: Call nearest hub site to get referral and more information about the program or complete the interest form on the website and someone will reach out and get the provider formally enrolled
