Program Delivery
Eligibility: Patients of Salinas Valley Health, regardless of age, with a need for health education, social support, or care navigation assistance. Individuals who may face barriers to care such as language, transportation, or socioeconomic challenges. Patients with chronic conditions, frequent hospital or ED visits, or complex care needs.
Languages: Interpreter services available, Spanish, English
