California Integrated Care Management (CICM) is a person-centered Medi-Cal managed care benefit. Provides high-touch coordination for members with complex medical, behavioral, and social needs. CICM assigns a single lead care manager to unify physical behavioral, and community-based services.

Core Goals:

  • Coordinate care across medical, mental health, and dental systems.
  • Address social determinants of health like housing and food security.
  • Reduce preventable emergency room visits and hospital stays.
  • Prevent duplication of medical services.
