Description

Runs a confidential health survey to help CDPH learn about how current and former residents find and use healthcare services. Survey results are used to improve health services in the community.

Participants are chosen at random, and those who choose to participate can receive a $50 - $75 gift card as a token of appreciation. Individuals cannot volunteer to take the survey.

The program does not serve San Francisco and Los Angeles Counties.

Eligibility: Current and former residents of California. Staff determine eligibility and choose participants.
Languages: Translation services available for multiple languages, American Sign Language Interpreter available on request, Spanish, English
Application Process: Chosen participants will be contacted by a representative via phone, mail, or email.

If the person qualifies and agrees to participate, they will be asked to provide their preferred date and time for an appointment. Appointments can also be conducted on the spot. Weekend and evening appointments are available.
