There are twelve approved service providers in California that offer the Federal Lifeline wireless phone service discounts.  Consumers call the companies and choose the plan that is most convenient for them.  The companies then apply for the discounts and provide service at a discounted rate to the consumer.  Only one discount per household for each residential address is allowed.  Failure to follow the one discount per household rule will cause a household to lose its discounts.

Application Process: 1. Consumer contacts phone company to begin the application process.
2. Phone company reviews eligibility rules with consumer
3. Consumer receives application form with a Personal Identification Number (PIN) in a PINK envelope in the mail within 3 weeks
4. Consumer completes application using Online Process or Paper Process
5. Consumer receives either an approval letter or a letter of disqualification

Consumers can only have one discount (landline or wireless) per household. Any violation to this rule can lead to disqualification and loss of discounts.

Documents Required: ◘ Front page only of prior year's state (540, 540A, 540 2EZ, 540NR, or 540X), federal (1040, 1040A, 1040EZ, 1040NR, 1040NR-EZ, 1040SS, or 1040X) or tribal tax return ◘ Income statements or paycheck stubs for 3 consecutive months within the past 12 months ◘ Statement of benefits from Social Security, Veterans Administration, retirement/pension, Unemployment Compensation, and/or Workmen's Compensation. ◘ Alimony and/or child support documents, and/or ◘ Other official documents
