Description
Concerned family members of potentially unsafe elderly drivers can contact the DMV Medical Review Division to request a medical review of the driver. Family members get an affidavit from DMV, which must be completed and notarized, then returned to the DMV. DMV will contact the person named in the affidavit and advise them that their abilities are being reviewed, and will provide them with medical forms to be completed by the person's physician, and filed with DMV for review. Person being reviewed may submit a written request for a copy of their medical file, from DMV.
