Description
Behavioral Health Care program provides a cost-effective longitudinal “home” to facilitate access to an array of behavioral health care, medical care, and community-based social services and supports for adults with serious mental illness and chronic health conditions.  BHH is an integrated healthcare service delivery model that is recovery-oriented, person and family centered which assists with a better patient experience and better outcomes than those achieved in traditional services, due to the care coordination it provides.  BHH Designated Provider Agencies provide six core BHH services to eligible participants: comprehensive care transition; health promotion and education; individual and family support; referral to community supports.

Program Delivery
Eligibility: Person must have Medicaid, must have a serious mental illness diagnosis and must have Medicaid claims in a calendar year that exceed $10,000; Interested participants may call the BHH customer service center to verify eligibility; if determined to be eligible, participants will be referred to the BHH Designated Provider Agency of their choice
