Disability Hub MN
: A free statewide resource network that helps individuals solve problems, navigate the system, and plan for their future. Visit their COVID-19 resource page for more information and resources or call (866)333-2466.

Disability Information and Access Line (DIAL): A national hotline to connect people with disabilities to information and services to improve access to COVID-19 vaccines. The hotline is available to help people with disabilities find vaccination locations in their communities, assist callers with making vaccination appointments, and connect callers to local services – such as accessible transportation – to overcome barriers to vaccination. The hotline also can provide information and resources to answer questions and address concerns about the vaccines and can connect callers to information and services that promote independent living and address fundamental needs, such as food, housing, and transportation.

The Partnership for Inclusive Disaster Strategies Disaster Hotline: Provides information, referrals, guidance, technical assistance and resources to people with disabilities, their families and others seeking assistance with immediate and urgent disaster-related needs. Call (800)626-4959 or email info@disasterstrategies.org 24 hours/7 days a week. Calls will be returned within 24 hours.

Senior Related Resources

Community Healthcare Services: Provides tele-health and ongoing care coordination services assistance to seniors for free.Visit website for additional information.

HelpOlderAdultsMN.org: Resources for older adults during COVID-19. Information includes topics on adult day services, care coordination, chores and homemaking, meal delivery, transportation, and more.

Senior LinkAge Line: Older adults and their families can call1-800-333-2433 to find resources and make connctions with alternative services, if someone's provider or support network has changed or temporarily stopped providing services. Individuals on Elderly Waiver, Alternative Care or Essential Community Supports, should contact their case manager/care coordinator to discuss their needs first. Resources include home-delivered meals; grocery or prescription drug delivery; help in the home; transportation; caregiver consultants and respite services.
