|
SOCIAL SERVICES TO ADULTS: Provides comprehensive assessment and case management to elderly and vulnerable adults who are seeking to remain/become self-supporting and self-sufficient. Works to prevent abuse, neglect, or exploitation. Reduces unnecessary institutionalization. Secures appropriate care when necessary. Programs include: - IN HOME AIDE SERVICES (IHAS): Secures medical supplies. Assists with bathing, dressing, transferring, grooming, and eating. Handles light chores - meal preparation, laundry, housekeeping, and shopping - to help resident remain in-home and as independent as possible.
- PROJECT HOME or C.A.R.E. (Certified Adult Residential Environment): Adult foster care family for persons with mental illness or other disabilities. Documentation of disability, income, and functional capability will be required. Residents must be able to self-medicate. Providers are screened and certified to provide room, board, assistance with ADL's, recreational activities, medication supervision, and assistance with transportation to medical appointments.
- RESPITE CARE PROGRAM: Short-term, temporary care to provide a break for family caregivers. Available at planned intervals, in a time of crisis, or on an as-needed basis. Serves children and adults with developmental disabilities and adults with functional disabilities.
- ADULT PUBLIC GUARDIANSHIP: Legal process mandated by court, if a person's ability to make health and safety decisions for themselves is impaired by disease, accident, or disability. Court authorizes/appoints a person or agency to make decisions for the individual's health care, shelter, daily needs and/or their property. Each case is overseen by a Review Board.
- PROJECT S.A.F.E. (Stop Adult Financial Exploitation): Educates individuals and organizations to prevent and remedy the financial exploitation of vulnerable adults (wrongfully taking or using an older adult's funds/property through theft, scams, fraud, or predatory lending).
