A social worker and a case associate provide services to families whose children are at risk of out-of-home placement due to: 1) crisis related to ongoing physical or mental illness of a family member, 2) environmental crisis, 3) high risk of child maltreatment, 4) needing intensive services after reuniting from a foster care placement, or 5) families ordered to this program due to abuse or neglect
