Documents Required	Proof of income (4 current pay stubs; signed 1040 tax form (1st two pages); public assistance letter; Social Security Income; unemployment verification; or verification of wages signed by employer). If the child has a disability a copy of their most recent IEP must be submitted.



Volunteer Requirements	16 and older.
