ASSERTIVE COMMUNITY TREATMENT (ACT): Provides mental health services to persons who are unable or unwilling to access traditional, facility-based services. A team of mental health workers deliver services wherever the consumer is willing to receive them. Designed for individuals who need 24-hour support and monitoring but not in a hospital setting. Services include diagnostic evaluation, crisis intervention, case management, medication management, housing support, and transition to other needed services.
