Description
|
MEDICAL TRANSPORTATION: Provides non-emergency medical transportation to individuals receiving Medicaid benefits and in need of transportation to a medical appointment only. To receive transportation for local (within county) appointments, call at least 48 hours in advance. Individuals requesting to be transported out of the county, must call at least 72 hours before the appointment. Transportation to and from appointments are provided by Delmarva Community Transit (D.C.T.).
Provide the following information when calling to make an appointment:
-Medicaid Number
-Name/address of doctor, clinic, or Primary Care Physician (PCP)
-Appointment and return time
-If anyone will be accompanying you and if you are a wheelchair patient.
