Eligibility	Open to all Medi-Cal or Medicare recipient, or the uninsured.

Languages Offered	English and Spanish. Additional languages and ASL are provided via a translation service.
Program Fees	No fees with certain health insurance plans. Sliding scale based on family size and income, and program coverage guidelines.

Documents Required	Photo ID, medical insurance card for those insured, verification of income, and payment if applicable
