Description

Qualified individuals, according to poverty guidelines, can pay only a percentage of their utility bill based on their income: *for electric bills, customers pay 10% of the bill, for gas and electric, customers pay 5% of their income on each bill.

Starting November 1st HEAP/PIPP will be at Ohio Means Jobs for Winter Crisis. This will be for two weeks. After that they will be back in the State Road Office.

Hours Of Operation Satellite Sites and Hours by appt only as follows: HEAP appointments will be available at the following locations by appointment only. Twice a month at Country Neighbor in Orwell from 9am - 3pm. The second Tuesday of the month at the Geneva Public Library from 9am - 1pm. Every Wednesday they will be at the Andover Country Neighbor from 10 am - 1 pm. First Thursday of the month will be at Jefferson Henderson Memorial Public Library from 9 am - 3 pm. Every other Thursday will be at Conneaut Resource Center from 9 am - 3 pm. Please call 440-983-7405 for site availability and to schedule an appointment. Documents Required ACCAA prefers that clients call the office about how to get their documents to the utility assistance program. Prefer materials to be brought in at time of appointment. They can fax to 440-997-6022 or email [email protected]. Proof of citizenship for everyone in the house: Either Birth Certificate or Social Security Card with birthdate (If children in the household without their parent, or if split household will need custody papers) Last 30 days of income UNLESS no income, seasonal, or self-employed: Seasonal - last 12 months of income from application date. Self-employed - Current Taxes. No income - will need a Letter of Support from a friend/family member who is supporting them stating amount given. Must be signed by friend/family member who is helping out. Will need friends address, phone number, and date as well. Utility bill: Main Heating Bill, Electric bill If applying for LIHWAP: Water and/or waste water bill If renting: need the landlord's name, address, phone number
