Eligibility	Low income and no insurance.



Application Process	Complete an application. You may pick up application at Department of Human Resources.
Documents Required	The following must be provided in order complete an application: Proof of Income (2 months of paycheck stubs, proof of SSI or SS, Unemployment, etc.) for everyone in household; Photo ID and SS Card; Up-to-date Food Stamp report (if applicable); Proof of any kind of financial assistance (ex. Section 8); Copies of receipts from medical bills you pay monthly; Copies of all insurance cards; Copies of all bills paid to show proof of monthly expenses.
