Documents Required Drivers License or State Issued ID, hardship letter (written by client), copy of last 3 pay stubs, for rental assistance you must have a copy of your current lease and for utility assistance you need the original bill (no copies) and a 6 month Activity Statement (get this from your utility company) APPLICATION CAN ONLY BE SUBMITTED IF YOU HAVE ALL OF YOUR DOCUMENTATION.
