Description	OVERVIEW: A structured day treatment program for adults 18+ with psychiatric disorders, as well as co-occurring substance use disorders, who require a higher level of care than outpatient treatment. Program provides treatment as a step-down from inpatient hospitalization and can sometimes prevent hospitalization. Patients work collaboratively with psychiatrists, social workers, and nurses who provide assessment, treatment planning, medication management, group & individual therapy, family therapy/psychoeducation, transition and aftercare planning, and case management services. PARTIAL HOSPITALIZATION PROGRAM (PHP): 5 days a week, 6.5 hours per day. Mon-Fri 9AM-3:30PM. The average length of stay is 3 weeks. Most patients start at the PHP level of care and, as their clinical symptoms improve, step down to the IOP level of care. INTENSIVE OUTPATIENT PROGRAM (IOP): 3-5 days/week for 3 hours per day. This level of care typically serves as a stepdown from a PHP level of care.

Program
Eligibility	Adults 18 and older who do not require an inpatient setting but need more intensive treatment than outpatient office visits. Referral from a licensed clinician is required.


Program Fees	Medical Assistance, Medicare, and most major insurance plans.
Application Process	Must be referred by community providers, FMH emergency department, or stepped down from the Behavioral Health Unit or other hospital inpatient psychiatric setting.
Documents Required	Clinical documentation from referral source to support PPHP level of care.
