Eligibility	Must meet ALL of the following criteria: Must be 18 years of age or older with physical or intellectual disability Not receiving services from Department of Developmental Services or Department of Rehabilitation Not receiving case management under the Department of Mental Health or Multi-purpose Senior Services Program At risk of being homeless

Languages Offered	Additional languages are provided via a translation service
