NETWORK OF CARE: Provides a website that offers community health data and resources in an easy-to-read format. The resources offers individuals, families and the county with a searchable directory of all health services on a federal and local level. The site is designed to improve access to services in Worcester County and provide tools to improve health decision making for all people. There is also a specialized interactive, educational tool kit including information on fitness, healthy weight, diabetes and becoming smoke free. The personal health record on the site provides individuals with a space to organize and store important medical and healthcare related information.
